Diabetes Mellitus Algorithm S: Screening and Prevention

Adult person age greater than 45, or adult with hypertension, dyslipidemia or multiple recognized risk factor [A]
Obtain fasting plasma glucose *
Is fasting plasma glucose [FPG] greater than or equal to 126 mg/dL? [A] Box 3 yes to box 4 Confirmed FPG greater than or equal to 126 mg/dL in 2 random FPG samples
Is fasting plasma glucose [FPG] greater than or equal to  110 mg/dL or other risk factor for DM? [B] Counsel for intervention to prevent DM [B] Diagnosis of Diabetes. Go to Module D
Box 8 no to box 7 Repeat screening every 1-3 years

* Note: Random non-fasting plasma glucose is not recommended as first line screening. However, non-fasting plasma glucose >200 mg/dl (on at least two occasions) is sufficient to diagnose DM, and <110 mg/dl is sufficient to exclude it. Random (non-fasting) plasma glucose in the range 111-199 mg/dl should be followed up with a fasting plasma glucose.
