   This reminder is due every 6 months for patients whose last DBP was >89

    or the last SBP was > 139 and the patient has a diagnosis of HTN. 

    The reminder becomes not applicable if a BP <140/90 is recorded. 

    The reminder is resolved for 6 months by: 1. recording the education

    topic for HTN LIFESTYLE EDUCATION 2. recording that an intervention has

    been done - adjusted meds or specific recommendations 3. recording that

    the patient declines an intervention

    Any of the following interventions will resolve the reminder for 6

    months: Medication   (reminder term HTN MEDICATION INTERVENTIONS)

      HTN MEDICATIONS ADJUSTED              (HF)

      HTN MED CHANGES NOT NEEDED            (HF)

    The reminder becomes not applicable for 12 months if the health factor of

    INCORRECT HTN DIAGNOSIS is entered.
