Reminder Description:

    Patients with a diagnosis of HTN should be counseled at least yearly on

    the life style modifications which may improve blood pressure control. 

    

    This reminder will display for a given patient if they have a diagnosis

    of HTN in the past 18 months and if their last BP is less than 140/90. 

    

    The reminder will not display if the last DBP was >90 or if the last SBP

    was >140 because the reminder for elevated BP will be due and the

    educational intervention can be addressed using that reminder. 

    

    The reminder term for HTN EXERCISE contains the education topics for

    exercise and a HF for patients who already exercise adequately and one

    for those patient who have a contraindication to exercise. 

    

    The reminder term for HTN NUTRITION contains the education topics for

    nutrition and a health factor for refusal of referral to dietetics and

    one for those who already follow a heart healthy diet. 

    

    Both of these reminder terms are required to resolve the reminder.  And

    both of these reminder terms contain the education topic HTN LIFESTYLE

    EDUCATION so that entry of this topic will resolve the reminder. 

    

    The reminder term HTN NO EDUC INTERVENTION WARRANTED contains a health

    factor of the same name.  Based on the patient's comorbid illnesses and

    life expectancy, this educational intervention may not be appropriate. 

    This health factor renders the reminder not applicable for 6 months. 

    

    The reminder term HTN INCORRECT DIAGNOSIS contains a health factor of the

    same name.  This health factor makes this reminder not applicable for 18

    months.
