  1. Patient With Known Ischemic Heart Disease (IHD)

  2. Obtain Focused History, Physical Exam, And Review Medication And Reversible Risk Factors

  3. Are There Sudden Or Acute Changes In Symptoms Or Other Clinical Findings To Suggest An Acute Coronary Syndrome (ACS)?

  4. Are Stable Angina Symptoms Adequately Controlled?

  5. Has The Patient Been Risk-Stratified?

  6. Are There Indications For The Assessment Of Left Ventricular Function (LVF) (e.g., Signs Or Symptoms Of Congestive Heart Failure (CHF)?

  7. Assess LV Function, If Indicated

  8. Moderate Or Severe Left Ventricular Dysfunction (LVEF <0.40)

  9. Ensure Therapy For CHF/LV Dysfunction

  10. Are There Any Indications For Referral To Cardiology?

  11. Is Patient At High Risk For Sudden Cardiac Death?

  12. Continue Beta Adrenergic Blocking Agent And ASA

  13. Does Patient Have Low Density Lipoproteins-Cholesterol (LDL-C) >100 mg/dL Or High Density Lipoproteins (HDL) <40 mg/dL?

  14. Is Patient Hypertensive (Blood Pressure > 140/90)?

  15. Is Patient Currently Smoking?

  16. Does Patient Have Diabetes Mellitus (DM)?

  17. Does Patient Screen Positive For Depression?

  18. Adjust Angina Management, If Indicated

  19. Consider Exercise Rehabilitation Program

  20. Schedule Regular Follow-Up
